[MMTK] PDF version of the MMTK manual

I got several requests for a PDF version of the MMTK 2 manual, so I
put one on my website (follow the link at
http://dirac.cnrs-orleans.fr/MMTK/). I'll still work on the
formatting, and do some proofreading, so don't waste your printer
paper yet unless you really need hardcopy immediately.
